Phoenix Rising Treatment holds two licenses through the Utah Department of Health and Human Services (DHHS).

Joint Commission Accreditation

Phoenix Rising Treatment is currently pursuing Joint Commission accreditation. We are not yet accredited. We will update this page the moment that status changes, with documentation available on request.

Our application has been submitted; we expect to hear back within 45–90 days.

Staff Licensure & Credentials

Our clinical team includes LCSW, CMHC, and SUDC-credentialed staff, supervised per Utah licensing requirements. Full bios and individual credentials are listed on our Staff page.
